Job Description:
- Conduct thorough initial evaluations with patients.
- Ensure every patient receives comprehensive and individualized behavioral health sessions.
- Provide continuous communication with the team and referring physicians.
- Implement health treatment programs based on each patient's particular needs focusing on diet change.
- Accurate and timely documentation of patient encounters, counseling sessions, progress and discharge notes.
- Assist in marketing to new and existing referral sources
